GENERAL AFFIDAVIT.

State of Tennessee County of Bradley, SS:

In the matter of P.C. of Eva Lee minor child of David G. Cooper, No. 704338 

On this 26 day of Febry 1900 A.D. personally came before me, a County Court Clerk in and for the aforesaid County, duly authorized to administer oaths, Dr. Geo. M. Bazemore aged 64 years resident of Cleveland in the County of Bradley and State of Tennessee, well known to me to be reputable and entitled to credit, and who being duly sworn declare in relation to the aforesaid case as follows:

I visited the wife of said David G. Cooper about three miles West of Cleveland Tennessee and attended her in child-birth on the 12th day of Septr in 1887.  according to the date of said visit now on my Book though the child then born may have been born after midnight of the 12th and I think that is probably correct which by my account would make the date of birth Septr 13, 1887.  I am informed that it was the youngest child of said parents, and I do not doubt that it is the same child as the claimant above named. 

I attended as physician at the birth and the date for visit to the wife of "David cooper" now appears on my Book as stated.

I further declare that I have no interest in said case and am not concerned in its prosecution.
